The Science of Lip Aging: What happens to your lips after 40?
For many people, the first signs of aging appear on the lips. Maybe your lipstick no longer applies as smoothly. Your lip liner begins to feather. Your lips feel perpetually dry, even after applying balm. Or perhaps you've noticed that your lips simply don't look as full or defined as they once did. These aren't isolated concerns. They're the result of predictable biological changes that occur as we age. Understanding why lips age differently than the rest of the face is the first step toward treating them effectively.
Why do your lips age faster than the rest of your face?
The skin on your lips is unlike skin anywhere else on your body. The vermilion - the visible pink portion of the lips - is made up of only three to five cellular layers, compared to up to sixteen layers across much of the face. Lips also lack oil glands and sweat glands, making them far less capable of retaining moisture naturally. Because the skin is so thin, the underlying blood vessels show through, giving lips their natural pink color. But that thinness also makes them especially vulnerable to aging, environmental damage, and dehydration. Over time, these structural differences become increasingly noticeable. Lip aging isn't caused by one factor, it's the result of several biological processes occurring simultaneously.
Beginning in our twenties, collagen production decreases by approximately 1% each year. By our forties and fifties, that gradual decline becomes much more visible. Collagen provides the structural framework that gives lips firmness, definition, and resilience. As collagen diminishes, lips lose support and become softer, thinner, and more prone to wrinkles.
Hyaluronic acid is naturally present within the lips and surrounding tissue, helping attract and retain water. As natural levels decline with age, lips become less hydrated, less supple, and lose some of their youthful volume. Unlike temporary dehydration, this represents a structural change within the tissue itself.
Just beneath the lips sits a layer of subcutaneous fat that contributes to their youthful fullness. With age, these fat pads gradually shrink and redistribute. The result is a flatter appearance, decreased projection, and reduced overall volume.
One of the most overlooked changes is the gradual loss of definition along the vermilion border, the crisp edge separating the lips from the surrounding skin. As collagen, elastin, and structural support decline, this border becomes less distinct, making lipstick more likely to feather while reducing the appearance of naturally defined lips.
Repeated facial movement from speaking, smiling, drinking, and expressing emotion creates tiny folds within the skin over decades. As collagen and elasticity decline, these temporary folds become permanent vertical lines, often called "barcode lines." These wrinkles extend above the lips and become more noticeable when lipstick settles into them.
What can we do about it?Â
Traditional lip balms are designed primarily to protect the surface of the lips. Most rely on waxes, petrolatum, or occlusive ingredients that temporarily seal in moisture and create the feeling of hydration. While helpful for environmental dryness, they do not address the biological changes responsible for aging lips. This is why many people find themselves constantly reapplying balm without seeing lasting improvement.
As our understanding of skin biology has evolved, so has the way aesthetic physicians approach lip rejuvenation. Rather than simply creating temporary swelling or coating the surface, today's regenerative philosophy focuses on supporting the skin's own ability to rebuild and repair. This concept, known as bioremodeling, has become central to many in-office aesthetic treatments.
Introducing LipLift: a Bioremodeling Approach
Dr. Diamond's Metacine LipLift Hyaluron Peptide-6 Biofill was influenced by the principles of facial bioremodeling practiced in Dr. Diamond’s practice, and was developed to address multiple signs of lip aging simultaneously through bioengineered ingredients designed to support healthier, younger-looking lips. Inspired by the Diamond Facial Contouring Procedure, LipLift is the latest launch in our Contour Collection. Rather than functioning like a gloss or temporary plumper, LipLift was engineered to visibly improve the biological characteristics of aging lips by helping support the main key markers including lip volume, vermilion border definition, fine lines around the mouth and natural lip color. Its formula combines advanced peptide technology, topical hyaluronic acid filling technology, and bioidentical barrier lipids to visibly restore lips without needles or irritation.
At the heart of LipLift is the Hyaluron Peptide-6 Biofill Complex, a proprietary blend of six advanced bio-peptides engineered to address the structural changes that occur as lips age. The complex works by supporting the skin's natural production of collagen, hyaluronic acid, and adipose (fat) tissue, the very components responsible for youthful lip volume, definition, and contour. At the same time, it helps inhibit lipolysis, the natural breakdown of fat that contributes to thinning lips over time.
The formula is paired with Acetyl Hexapeptide-8, a next-generation neuropeptide that visibly softens expression-related creases, smooths vertical vermillion lines, and helps lips appear firmer and more refined.
A topical filling system featuring hyaluronic acid, castor seed oil, and GlycoHyaluronic Filling Spheres draws water beneath the skin's surface to visibly plump and smooth lips without the tingling sensation associated with traditional lip plumpers.
Bioidentical barrier lipids, including ceramides, essential fatty acids, and shea butter, restore moisture while strengthening the lip barrier for lasting comfort.

Healthy Lips Speak Volumes
Beautiful lips aren't simply fuller, they're healthier. They have strong barrier function. They retain moisture naturally. They maintain crisp definition. Their surface is smooth. Their color is vibrant. Their structure is supported from within.
Treating lips like an extension of skincare, not simply makeup, allows us to address the underlying causes of lip aging rather than temporarily masking them. The future of lip care isn't about creating bigger lips; it's about restoring healthier ones.
